2. Includes both inpatients and outpatients.
3. Co-Trimoxazole is Sulfamethoxazole plus Trimethoprim.
4. Gentamicin synergy.
5. Bacteroides were tested against piperacillln; percent susceptible to piperacillin
/ tazobactam may be higher.
6. Includes 1 vancomycin-intermediate strain of S. haemolyticus.
