1. Number of organisms tested may include duplicate isolates from the same patient.

2. Both inpatients and outpatient data were included for S. pneumoniae and H. influenzae. Beta-lactamase negative strains of H. influenzae from non-sterile sites are not included. Data for all other organisms reflect inpatients only.

3. Can substitute doxycycline 100mg IV Q12h
